Abstract

Introduction: Hypertension affects nearly half of the population in the United States and, if left untreated, can precipitate chronic and life-threatening conditions. Self-measured blood pressure (SMBP) monitoring is an effective solution to diagnosing and managing blood pressure. The aim of this QI project was to improve hypertension management in a Midwest urban healthcare clinic by implementing an SMBP program. Methods: A quality improvement project and evaluation of the implementation of an SMBP program in a small urban Midwest healthcare clinic. Results: A total of 32 patients received SMBP monitors, 25 patients were included in the final data analysis, and the BP logs of 12 patients were collected. Patients without insurance may be more likely to return with their BP log. SMBP can expedite diagnoses and changes in treatment management. Conclusion: Insurance status may not indicate patients' motivation to take positive steps toward their health. Patients may be diagnosed sooner, or providers may make medication adjustments earlier for patients using SMBP.
