Abstract
Background:
Thumb carpometacarpal (CMC) osteoarthritis (OA) is a progressive degenerative condition that impairs hand function and occupational performance, particularly among older adults. Non-surgical interventions emphasizing dynamic stability—such as proprioceptive training, manual therapy, strengthening, and orthosis use are increasingly implemented in rehabilitation practice. However, there is variation in how these interventions are applied and measured, creating ambiguity in treatment planning.
Objective:
This scoping review systematically mapped and summarized the range of dynamic stability interventions used to manage thumb CMC OA and identified the outcome measures employed to evaluate pain and function.
Methods:
The review followed Joanna Briggs Institute methodology and PRISMA-ScR reporting guidelines. The Population-Concept-Context (PCC) framework guided inclusion criteria. A systematic search was conducted across PubMed, and CINAHL. Screening, data extraction, and management were completed using Covidence software. Extracted data were thematically synthesized to identify intervention types, outcome measures, and evidence gaps.
Results:
Data were extracted from n = 26 studies published between 2011 and 2024. Most were randomized controlled trials. Interventions included joint mobilization, proprioceptive training, dynamic stabilization, isometric and neuromuscular control, and orthotic approaches. Commonly reported outcomes were pain, grip and pinch strength, and functional status. Visual analog and numeric pain scales, dynamometers, and the QuickDASH were the most frequently used instruments. Most studies reported short-term improvements in pain and function, although few assessed outcomes beyond three months. Common limitations included small sample sizes and brief follow-up durations.
Conclusions and Implications:
Findings support the effectiveness of conservative, non-surgical strategies emphasizing dynamic stability for managing thumb CMC OA. Results highlight the need for standardized intervention protocols, consistent outcome measures, and longer-term evaluations to enhance evidence translation within occupational therapy practice.
